Social Care Support Worker - Transitons

Location: Hull

Hours: 37.5 hours per week

Salary: £18,915 per annum + benefits and expenses

Closing Date: Ongoing

We’re looking for empathetic and resilient Senior Support Care Workers to join our new Transitions team.  You’ll be passionate about care and deeply committed to helping people with disabilities who can present with challenging behaviour.

Our Transition Care Worker position is a brand-new role to facilitate the transition of new complex referrals into our services.  You will be supporting the transition of individuals with complex needs including Autism, mental health conditions or other disabilities.

This role will involve working with individuals in their current service provision and providing support to transition into their new accommodation as well as ongoing assessment of the individual to ensure the best possible care and support is offered at all times.

What do we expect from you?

• NVQ Level 3 in Social Care qualification

• Experience of challenging behaviour and working with highly complex cases

• Relevant experience working with transitions and setting up new services would be ideal

• Compassionate and empathetic

• An interest in people and a commitment to helping others

• Good IT skills

• Full driving licence and access to a vehicle is essential as the role will involve some travel
